The information you provide assists Dog Tag Buddies in assessing the appropriateness of the patient for our program. Similarly, it helps us determine their individual needs when selecting an emotional support animal or service dog for the patient. For that reason this form is a requirement in our application process to consider the veteran for the our program.
Also, the form uses a 3rd party application, Apricot by Social Solutions Global, Inc., whose controls and practices meet or exceed HIPAA requirements.
